1. Purpose of This Policy
This Injury Claim Reporting Policy outlines the formal process for reporting, documenting, and investigating any injury claim related to PersonalHour equipment or services. The purpose is to ensure all claims are handled legally, ethically, confidentially, and with full transparency according to Ohio law and internal corporate standards.
2. Reporting an Injury – Mandatory Requirements
To ensure that every injury claim is properly documented and reviewed under legal supervision, the reporting party must follow all steps below.
Step 1 — Submit Initial Report
The injured party or representative must submit an initial written report containing:

Full name and contact information

Date, time, and location of incident

Description of the injury

Description of the equipment involved

Any witnesses, if applicable
Step 2 — Provide Required Evidence
To open an official claim, the reporting party must provide:

Photos of the equipment (PersonalHour Team, will ask few questions to collect the right photos)

Videos of the incident or post-incident condition
-
A detailed timeline
-
Any medical documentation (if available)
-
A statement confirming all information is accurate to the best of their knowledge
Step 3 — Video Call Verification
A video call with our team may be required to verify the condition of the equipment, the location, and the injury description.
Step 4 — On-Site Investigation (If Needed)
For more complex cases, a visit from one of PersonalHour’s certified investigators may be required.
The reporting party must provide reasonable access to the equipment and the location of the incident.
3. Oversight and Handling of Claims
Every injury claim will be legally supervised and reviewed by the following internal teams:

A member of PersonalHour’s Legal Team

Head of Engineering

Chief Operating Officer (COO)

The CEO may be involved when necessary
All steps of the investigation will be documented and maintained in compliance with Ohio safety and consumer protection laws.
4. Timelines for Response
To ensure timely handling:

Both PersonalHour and the claimant agree to respond within three (3) business days to any request for information, documentation, evidence, or clarification.

Failure to respond within the required timeframe may delay or suspend the claim.
5. Confidentiality Requirements
All injury claims must remain strictly confidential while under investigation.
Prohibited Actions:

Sharing any information, images, videos, or communications about the claim

Posting claim details on social media

Sending claim information to third parties without legal authorization
Legal Basis
Improper disclosure or distribution of claim information violates internal policy and may violate Ohio Revised Code §2917.21 (Telecommunications Harassment / Unauthorized Disclosure).
Violation Consequence:
If any part of the claim is shared publicly or with unauthorized parties, the claim will be automatically rejected and closed.
6. False or Fraudulent Claims
Submitting false, misleading, or intentionally incomplete information is strictly prohibited.
Legal Basis:
False claims may violate Ohio Revised Code §2921.13 (Falsification).
Liability for False Claims:
If a claim is proven false with sufficient evidence:

7. Valid Claims & Compensation
If an injury claim is found to be valid and supported with clear evidence, PersonalHour will:

Provide fair compensation or remedy based on the specifics of the case

Execute corrective actions if equipment evaluation indicates a manufacturing or design issue
Compensation may include reimbursement, replacement, repair, medical support, or other remedies determined by Legal and Engineering.
8. Agreement to Policy
By submitting an injury claim to PersonalHour, the reporting party acknowledges and agrees to:

Participate in the investigation

Provide full truthful documentation

Maintain confidentiality

Accept the findings of the supervised investigative process

Receive compensation only after the claim is fully validated
